Player Forms

Dragonfly forms and verification of player eligibility must be completed before players may participate in pre-season conditioning, try-outs, practice and play.

The deadline to submit Dragonfly forms is January 6, 2025.

1. PLAYER INFORMATION FORM >>> 
   New and returning players complete this form.
   
   
2. MEDICAL >>>
   GHSA physical exam and medical eligibility forms are required each year prior to participating in pre-season conditioning, try-outs, practice and play. ​Dekalb County uses DragonFly Max to automate the management of required athletic health forms. Learn more here. Instructions to set up a DragonFly account are here.
   
   Print the physical form and have it completed by a medical doctor, nurse practitioner, or  physician assistant. Upload completed form in DragonFly and submit the additional online forms. After submitting all forms in Dragonfly, notify LHS Athletic Director Tricia Newmyer at [email protected] to complete the eligibility confirmation process.

3. DUES >>>
   Player dues must be paid by January 2025 (DATE TBD). Dues include lacrosse bag (second year players) and shooter shirt. Dues also cover team helmet, home and away jerseys, and practice pennies to be returned to the team at the end of the season. Dues must be paid to receive helmet and uniforms.
   ​
   All other gear (chest/shoulder/elbow pads, gloves and sticks) is purchased by the players.  IMPORTANT: Chest protector must meet NOCSAE standard ND200. Learn more here.
